PSA Form Fill Up: Filing a PAN Application
File a PAN application from your PSA login: a preparation sequence, what to collect first, fees UTIITSL lists and what to send support if a screen fails.
A customer sits down at your counter with a phone full of photos and a half-remembered Aadhaar number. You’ve got the PSA login open. The pressure is to start typing, because everyone is waiting. But the first few minutes decide whether the application goes through cleanly or comes back as a problem a week later.
The good news is that most mistakes are easy to catch if you prepare in the right order. This post is a preparation guide, not a screen-by-screen walkthrough. The screens inside the SiOnline portal can look different from the official form, and we’d rather not describe buttons that might have moved. What we can do is tell you what to collect, what to check, and what UTIITSL’s own documents say.
What’s the order of work?
Here’s the short version. The rest of the post explains each step.
- Sign in to the SiOnline portal and check the wallet balance.
- Choose the form that matches the applicant.
- Enter the details in the order the form asks, using the checklist below.
- Read the whole application back to the customer.
- Follow the receiving channel’s submission and payment instructions.
- Keep the acknowledgement number and track the application with it.
UTIITSL’s General FAQs say an applicant can apply online on the portals of Protean (the other authorised PAN service provider) or UTIITSL, or at a PAN centre of either. So the official form exists in more than one place, and yours may look a little different.
Before you start: the portal and the wallet
On SiOnline’s current PAN agent login guide, UTI PSA (PAN Service Agent) agents work through the SiOnline portal, which is connected to the UTI portal. You don’t get direct UTI access. So if a UTI page refuses your SiOnline login, that isn’t a fault in your ID. Start from the link on the UTI PSA login page.
Check the wallet balance before the customer sits down. Each PAN application is deducted from it, because SiOnline provides a wallet-based ID. If the wallet is short, you’ll be stuck halfway through with a customer waiting. SiOnline’s wallet balance guide covers adding funds.
Which application type should you choose?
First question: does this person already have a PAN? If yes, don’t file a new application. Form 93 carries a declaration that the applicant does not possess a PAN, and the applicant is liable for legal consequences if that is incorrect. UTIITSL’s home page, checked on 18 October 2026, lists change or correction, reprint and e-PAN download as separate services from a new PAN. If your customer wants one of those instead, see our guide to lost or damaged PAN card reprints.
The same UTIITSL page says Forms 49A and 49AA are not accepted after 31 March 2026. For a first PAN, the Income Tax Department’s e-Filing FAQ, checked on 18 October 2026, says applications filed on or after 1 April 2026 use the new forms. UTIITSL’s General FAQs list them below. Check the current UTIITSL list before you choose one.
| Form | Applicant |
|---|---|
| Form 93 | Individual who is a citizen of India |
| Form 94 | Non-individual Indian entity |
| Form 95 | Individual who is not a citizen of India |
| Form 96 | Non-individual foreign entity |
We’ve written a separate explainer on Forms 93 to 96 if you want more on how they differ.
What details should you collect first?
Form 93 is a useful checklist even if your screen looks different. Collect these before you open the application, so you aren’t phoning the customer’s cousin for a date of birth halfway through.
- Name. First, middle and last name in full, without abbreviations or titles such as Shri or Dr. If the Aadhaar name has initials, the form wants the expanded name as well.
- Name as per Aadhaar. Copy it from the Aadhaar record.
- Gender and date of birth. Gender must be selected. The form shows the date as day, month, year.
- Aadhaar number. Anyone who has an Aadhaar number must quote it.
- Residence address. Mandatory: flat or building, road, post office, locality, district, state and PIN code. The office address is optional, except for a sole proprietor.
- Residential status and, for a non-resident or a resident but not ordinarily resident, the passport number.
- Mobile number and email address. Both are mandatory.
- Source of income. At least one option.
- Parents. Both parents’ names, unless the form’s single-parent option applies, and the parent whose name will print on the card.
- Assessing Officer (AO) code. The area code, AO type, range code and AO number. The form guidelines say the Income Tax Office and the UTIITSL and Protean service centres and websites can supply these.
- Representative assessee, a person who files for the applicant. The guidelines make this mandatory when the applicant is a minor, mentally ill or of unsound mind. UTIITSL’s General FAQs say the representative’s own identity and address proofs are needed too. Keep the child’s details and the representative’s details in separate groups.
- Communication address, the address the card is sent to.
Some information may be pre-filled. Check each pre-filled box against the documents. Treat the Aadhaar name and the full name as two separate items, and check the birth date by day, month and year, not just “does the year look right?”
What documents and photographs do you need?
UTIITSL’s General FAQs say every application needs proof of identity, proof of address and proof of date of birth. UTIITSL’s document checklist lists which documents serve for which proof. Don’t assume one document covers all three. A document that proves identity may not prove address or date of birth. If you want a plain-language walk-through, our post on documents by applicant type goes deeper.
The form also asks for two recent colour photographs on a white background, 4.5 cm by 3.5 cm, and a signature or left thumb impression across one of them. Form 93’s guidelines say a thumb impression must be attested by a Magistrate, Notary Public, Oath Commissioner or Gazetted Officer under official seal and stamp.
UTIITSL’s public guidelines describe the order for an applicant who files on its own website: fill in online, validate, submit, verify on screen, pay, print, affix photographs, sign in two places, attach copies of the proofs, then send the set to a UTIITSL office. Your SiOnline portal may order these steps differently, which is exactly why the next section matters.
Four questions to put to support before your first file
Send these in one WhatsApp message and keep the reply:
- In what order does the SiOnline portal take payment and uploads?
- Which submission method does my SiOnline UTI PSA account use, and what do I print or upload?
- What is deducted from my wallet per application, and does it include the government fee?
- Is PAN filing available in the SiOnline Android app?
Doing this once saves you from guessing every time.
How do you read it back, submit and pay?
UTIITSL’s General FAQs say edits are not allowed after final submission, and corrections go through a separate request after the PAN is allotted. That’s the reason for the read-back. Before you press the last button, read the whole application out to the applicant, including what they’ll get: a physical card and e-PAN, or e-PAN only.
Pay special attention to addresses. If an address proof spells the locality differently from what you’ve typed, don’t silently pick one. Ask the applicant which they want recorded. If the two can’t be matched, ask the receiving provider what evidence is required. Incomplete applications are treated as invalid, so fix any field the portal flags.
A few habits help here:
- Look at the Aadhaar name and full name one more time, as two items.
- Check the whole communication address, including locality and PIN.
- Make sure an earlier attempt isn’t still open in another window.
- Don’t assume closing a tab saved your work. Follow the portal’s own instructions for saving or submitting.
What happens at payment?
Two things matter at payment.
Your wallet. On SiOnline’s pages, each application is deducted from your wallet. Ask support in writing what is deducted per application and whether that includes the government fee below.
The government fee. As of 18 October 2026, UTIITSL’s General FAQs list these fees, inclusive of applicable taxes: ₹107 for a physical card dispatched in India and ₹1,017 for dispatch outside India. They list ₹72 for an e-PAN only (an electronic PAN, no physical card) submitted at a centre, and ₹66 for an e-PAN only submitted online through paperless modes (e-KYC with e-Sign, scanned e-Sign and scanned digital signature certificate). The FAQs do not list a separate fee for a physical card ordered through the paperless modes, so confirm the amount before you pay. Confirm the current fee on the official UTIITSL or Income Tax Department page.
If a payment fails or a debit looks wrong, keep the transaction reference and ask support to establish the payment state before you repeat the payment. Paying twice to “see if it works” is how people lose money.
What should you do after you submit?
Keep the acknowledgement or reference number for each file, and give the customer a copy before they leave. Keep the payment reference separately, in a different place or on a different line. UTIITSL’s General FAQs say an applicant can track an application with that number on the designated portal, and its home page lists a Track PAN Card option. We’ve written more on this in how to track a PAN application.
Don’t file a second application to see whether the first went through. Read the status first. If the application was rejected for incomplete or deficient information, UTIITSL’s General FAQs say it is treated as invalid, and a fresh application with correct details and the mandatory documents is needed. Our rejection reasons checklist shows the usual causes.
Say a customer comes back in a week
Here’s an example. Say a customer returns and asks, “What happened to my PAN?” If you’ve kept the acknowledgement number and the payment reference, you can open the status in a minute and say what it shows. If you’ve kept nothing, you can only say “I think I filed it.” The number is the difference between a quick answer and a long afternoon.
Can you do it from a phone?
On SiOnline’s PAN agent login guide, you can sign in from a phone. But PAN work involves scanning and uploading supporting documents, so a computer with a scanner is more practical. Ask support whether PAN filing is available in the SiOnline Android app before you rely on it.
If a screen fails: what to send support
Send these, and never a password:
- your login ID and the registered mobile number
- the wording on the screen, copied exactly
- the stage you reached, such as “after entering the Aadhaar number”
- the acknowledgement number, if one was issued
- the time you tried
If you send a screenshot, hide the customer’s Aadhaar number and photograph first. SiOnline says it never asks for a password or an OTP. Support answers by phone and WhatsApp, 24 hours, 7 days, on +91 91091 35959. Details are on the support page.
Questions agents ask
Which form do I use for a new PAN?
Form 93 is for an individual Indian citizen. Forms 94, 95 and 96 cover other applicants, as UTIITSL’s General FAQs list them.
Why can I log in but not submit?
Check the actual balance, the application record and the message on the screen. A failed submission can have more than one cause, so ask support before you repeat payment or final submission.
Can I edit an application after final submission?
No. UTIITSL’s General FAQs say edits are not allowed after final submission, and corrections go through a separate request after the PAN is allotted.
What if the customer already has a PAN?
Don’t file a new application. UTIITSL’s home page, checked on 18 October 2026, lists change or correction, reprint and e-PAN download as separate services.
